I haven't played THAT many games, but I'll say maybe the Gems from Harvest Moon:FoMT. especially the Kappa Gem (I think it is), because you can only get to the lake mine during the winter (unless you have the teleport stone). anyway, that's the only one I can think of right now.

Go left towards where the Gambler is located. When you get to Route 8, your start menu will automatically appear. Do not save the game. Instead, immediately press B to close the start menu. You will be attacked by a level 7 Mew. Catch it, then save the game.

You can also duplicate items in those red and bluepokemon games. His name starts with an M or something... Misino? I don't know...

Back to the topic: It's pretty difficult to get the Parade Kart in in Mario Kart: Double Dash (I just unlocked it today; totally worth the frustration ^_^ )

Rarest Items... Dpends really on what you mean by rare. Omnislash is incredibly hard to get, not rare because you can see it for yourself...

and i wanted to say that i didnt like the way camelot subestimated excalibur. its supposed to be the ultimate weapon for an ultimate warrior, like murasame/masamune in other games, but the Sol Blade is much stronger...

The item I found the hardest to get in any game would probably be the stupid Kikuichimonji in GS. I havn't played many RPG's, and no other genre really has 'rare' items.
Omg, and Entei in Silver, I spent so many hours of my life trying to get into the same route as him, and then hoping to encounter him, and then trying to catch him using swift balls or whatever they were called :D.
I then spent even more hours of my life training him up to level 89 :blink: :D. I had no life... :P

e.g Some monsters appear once or twice in 1 particular dungeon, and even then the odds of them dropping a certain item can be around 1 in 256 or sometimes higher.

They gave you the chance to get heaps of info about every monster, they then tease you by making them "drop" the info one in a blue moon! GAH!
